      Upon receipt of this application by Fitwize 4 Kids (FW4K)., I (we) hereby agree to accept the terms and conditions of the registration form contained in the following provisions;

      A deposit in the amount of $225 plus registration shall be presented with this registration form. The refundable until May 8th 2020 After this date and time, deposits are non-refundable. A signed and dated parent waiver is required for all camp participants. Campers will not be permitted to participate without a signed and dated waiver on file. Waivers are due on or before the first day of camp.
